LifeConnections Specialized Support Services

LifeConnections Specialized Support Services is a nonprofit organization that provides an array of youth and family driven solutions, including: individual and family therapy, targeted case management, mentoring, home-based treatment, and residential care.

LifeConnections Specialized Support Services is an enhanced family care residential service option for adults with intellectual and developmental disabilities and/or acquired brain disorders. Shared Living Providers may be families, individuals or couples supporting individuals in their home providing opportunities to learn new skills, socialize, participate in community activities, develop natural supports, and live a meaningful life in their community. 

Our careful, thoughtful, and personalized matching process ensures that there is a strong compatibility between each individual and their provider. We strive to achieve an individualized, customized living situation that promotes development of a strong foundation for meaningful and long-lasting relationships. All prospective providers must undergo an extensive background screening process and complete a thorough home assessment prior to being accepted into the program. Providers receive extensive training, case management, and access to an array of supports including: consultative services such as clinical, behavioral and nursing supports, respite, and 24-hour emergency on-call services. 
Our Shared Living program may include an array of service options tailored to each individual’s needs: 

Specialized Shared Living Services 

Specialized Shared Living is an alternative to the traditional shared living services and staffed residential service programs. It is designed to support individuals with complex medical and/or behavioral needs. This service incorporates residential staff support and embedded clinical services from our Clinical Intervention Support Team. 

Clinical Intervention Support Team Assessment Services 

Our Clinical Intervention Support Team Assessment Service utilizes a wraparound team-based planning model that is multi-faceted and individualized for individuals with complex medical and/or behavioral needs. Our Clinical Intervention Support Team includes an array of professionals with extensive experience working with individuals who have intellectual and developmental disabilities and/or acquired brain disorders. Our team includes a licensed psychologist, physician, psychiatric nurse practitioner, medical nurse practitioner, and a behavior analyst. The team engages the individual, providers, and staff to assess and monitor evolving needs, make recommendations, and create strategies to support the individual in a holistic and person-centered manner. 

Community Participation Services 

Community Participation Services are specifically tailored to provide individuals the opportunity to participate in integrated community activities. Our Community Integration Specialists assist individuals to increase independence, fulfill personal goals, learn or improve decision-making skills, safety skills and/or social skills. This may include a variety of community-based activities and volunteer opportunities that are inclusive and person-centered. 

Supported Employment Services 

Supported Employment Services offer assistance to individuals who would like to obtain competitive employment. Our Employment Specialists develop relationships with businesses within local communities and assist individuals in seeking employment in inclusive settings. Services may include assistance with resume building, job searches, interview skills, training, assistive technology, transportation and development of natural supports.
