Community Partners for Change, Inc. was organized to develop and implement a more personalized, consumer driven service program that meets the housing, vocational, educational, health and social needs of community members with disabilities such as developmental, mental illness, chronic substance abuse, dual diagnosis, Acquired Brain Disorder and other physical limitations.
Our main objective is to work with other service providers and caregivers to fill “SERVICE GAPS” that may exist in some of the services currently provided.
We believe that meaningful and long lasting positive change must come from True Community Partnership.
Residential Services
Residential support includes assistance and instruction to improve and maintain an individual’s skills in basic daily living, personal development, and community activities.
Home -2- Stay Support Services
Home -2- Stay support services, is one of our several programs that provide self-directed home based support services. The program provide comprehensive home-based services whose main objective is to allow individuals to continue living in their homes, to assist individuals maintain their independence, offer respite to family/care givers and minimize hospitalization.
Community Participation Service
All community participation services are designed and provided based on the individual’s specific needs, interests, competencies, and learning style, as described in the individual’s service agreement and personal profile.
