TruGranite Service Coordination is an independent, conflict free case management option for those eligible for the 1915(c) NH Developmental Disabilities (DD) Waiver or NH Acquired Brain Disorder (ABD) Waiver.

We will guide you to available services providers tailored to your goals, strengths and needs. We build relationships with your team to ensure proper supports and your satisfaction with services. Our goal is to ensure you live the life of your choosing, and help guide you to make choices and decisions.
